Application Cover Letter The application should contain a cover letter detailing the applicant’s name, designation, passport number, the trip’s intended purpose, and a declaration of who would be financially responsible for the trip. For both single and multiple-entry visa holders, it is mandatory to have health insurance that provides the following coverages while in Azerbaijan:

1. a) Costs associated with repatriation, urgent medical help, or urgent hospitalization.

b)Statements for the first stay in Azerbaijan and subsequent entries must be submitted for multiple entry visas.

1. c) The minimum amount of coverage must be at least 30,000 Euros.

If you’ve been tested for HIV, you must provide Proof of that on a certificate.
